What makes this achievement truly remarkable is not just the technology itself, but the dramatic chain of events that unfolded around it — including a multinational corporation’s attempt to buy out the company offering an exorbitant price, and thanks to the last-minute intervention by Tata Trusts and  Sridhar Vembu of Zoho that kept this critical technology in Indian hands.

The Global MedTech Monopoly: How a Handful of Giants Control the Market

A handful of multinational corporations control the world’s Magnetic Resonance Imaging (MRI) industry. According to a market study by the Competition Commission of India, the top five original equipment manufacturers — Siemens, GE, Philips, United Imaging and Hitachi — account for over 91 per cent of MRI market revenue in India. These companies are based in the United States, Germany, China and Japan.

This near-monopoly has had serious consequences for India. With no domestic alternative, Indian hospitals have had no choice but to pay whatever prices these global giants demanded. An imported 1.5-tesla MRI machine can cost anywhere between ₹3 crore and ₹4.5 crore. Add to that the high costs of installation, maintenance, and specialised infrastructure, and it becomes clear why MRI services remain out of reach for most Indians.

The numbers tell a stark story. India currently has only around 5,000 MRI machines, which translates to roughly 3.5 scanners per million people. For a country of 1.4 billion people, this is woefully inadequate. By comparison, developed nations have MRI densities many times higher. The lack of affordable diagnostics means that millions of Indians — particularly those in smaller towns and rural areas — cannot access the advanced imaging they need for early detection and treatment of serious conditions like cancer, neurological disorders and cardiovascular diseases.

India imports between 70 and 80 per cent of its medical devices by value. In financial terms, the numbers are staggering. India imported medical devices worth $8.1 billion (approximately ₹94,000 crore) in the financial year 2023-24, while exports stood at just $3.7 billion. This trade deficit of over $4 billion underscores the structural import dependency that has plagued the sector.

In the five years between FY2020-21 and FY2024-25, India spent over USD 25 billion in foreign exchange on imports of electro- medical equipment. For MRI-related products alone, the annual import bill is estimated at around ₹3,500 crore.

This dependency is not just an economic problem — it is a matter of national security and public health. When global supply chains are disrupted, as they were during the COVID-19 pandemic, India’s healthcare system becomes vulnerable. The pandemic exposed just how precarious this dependency is, and it served as a wake-up call for policymakers and industry leaders alike.

The government has recognised this challenge. The Production Linked Incentive (PLI) scheme for medical devices, with a budget of ₹3,500 crore, was launched to boost domestic manufacturing. More than 50 high-end medical devices have begun to be manufactured in India under this scheme. However, the overall import dependence figure has reduced dramatically. This is precisely why Voxel Grids’ breakthrough is so significant — it represents a genuine, home-grown solution to one of India’s most persistent healthcare challenges.

Voxel Grids’ 1.5 Tesla Revolution: A Technical Breakthrough with a Human Touch

The story of Voxel Grids begins with its founder, Sri Arjun Arunachalam, a 47-year-old electronics engineer who specialised in MRI physics at the University of Wisconsin-Madison. After spending nearly three years at GE Health Care’s research centre in America around 2008-09, Arunachalam returned to India with a dream — to build an MRI machine that was truly Indian.

What he and his team achieved over the next 12 years is nothing short of extraordinary. Voxel Grids has developed a 1.5-tesla MRI scanner that is not merely a copy of existing machines but incorporates original technological innovations.

What Makes It Different?

First, the Voxel Grids MRI scanner eliminates the need for liquid helium for cooling. Traditional MRI machines require large quantities of liquid helium, which is expensive, difficult to source, and requires specialised handling. By removing this requirement, Voxel Grids has made its scanner around 40 per cent cheaper to manufacture.

Second, the company has engineered highly compact and well-packaged electronics around the magnet without compromising the bore size. This makes the machine far more power-efficient and significantly reduces operating costs.

Third, the scanner is around 50 per cent lighter than conventional machines, making installation easier and reducing infrastructure requirements.

Fourth, and perhaps most importantly for patients, the Voxel Grids MRI scanner does not depend on costly air-conditioning infrastructure. This is a game-changer for smaller hospitals in India’s smaller cities and towns, where such infrastructure is often unavailable or prohibitively expensive.

Pay-Per-Use: Making MRI Accessible to All

Perhaps the most innovative aspect of Voxel Grids’ approach is its business model. The company offers pay-per-use pricing models, allowing hospitals to spread costs over time instead of paying a large upfront amount. This is particularly beneficial for smaller hospitals that struggle with the high capital expenditure associated with MRI machines.

This model could substantially expand India’s MRI footprint. Industry experts believe that the indigenous MRI scanner could eventually reduce the cost of an MRI scan by up to 70 per cent. This would make advanced diagnostic imaging affordable and accessible to millions of Indians who currently cannot afford it.

The MNC Acquisition Plot: How India Almost Lost Its Home-grown MRI

Building an MRI machine is not for the faint-hearted. It is one of the most complex and technologically demanding medical devices ever created, requiring years of research, enormous capital and expertise across physics, electronics, software and precision engineering.

Arunachalam and his team at Voxel Grids were running out of money while competing against corporations worth billions. Then, during the COVID-19 pandemic, a multinational medical technology company approached the startup with a lucrative acquisition offer.

It was a tempting offer. The financial uncertainty was immense, and the easy way out would have been to sell the company and walk away. But Arunachalam made a difficult decision — he rejected the deal despite huge financial provocation.

We thought partnering with the company would risk our innovation. We wanted to ensure that this technology remained India’s own,” he told India Today Digital. He viewed that partnering with a multinational would come at the cost of the very innovation he was trying to build.

It is common knowledge in the industry that most MNC giants offer to be partners and then gobble up their partners. By selling to a global corporation, India would have lost not just a company but the intellectual property, the technology, and the opportunity to build a domestic med tech manufacturing ecosystem at an affordable cost for commoners.

 The Tata-Zoho Masterstroke: Keeping India’s Innovation in Indian Hands

Just when things looked bleakest for Voxel Grids, two of India’s most respected institutions stepped in.

Tata Trusts, then led by the late Ratan Tata, provided crucial financial support that helped Voxel Grids continue its research and product development without giving up ownership. Later, Zoho founder Sridhar Vembu also backed the company, putting in ₹35 crore through Zoho simply out of gratitude for the motherland.

This funding was a lifeline. It allowed the start up to survive long enough to develop what could become India’s first indigenous MRI scanner. More importantly, it ensured that this critical technology remained in Indian hands.

The backing from Tata Trusts and Zoho is significant not just for what it achieved but for what it represents. It is a powerful statement that Indian institutions are willing to invest in deep-tech innovation, even when the risks are high, and the returns are uncertain. It is a testament to the vision of leaders like Ratan Tata and Sridhar Vembu, who understood that building a self-reliant India requires more than just policy — it requires courage, conviction and capital.

  A Game-Changer for Public Health

The implications of Voxel Grids’ breakthrough for public health in India are immense.

Lower Costs, Greater Access

The most immediate impact will be on cost. The Voxel Grids MRI scanner can be manufactured at nearly 40 per cent lower cost than imported alternatives and is around 50 per cent lighter. The company estimates that MRI scan costs could eventually fall by up to 70 per cent.

This is not just a marginal improvement — it is transformative. For a patient in a rural area who currently has to travel hundreds of kilometres to the nearest city for an MRI scan, a 70 per cent reduction in cost could mean the difference between getting a diagnosis and going without.

Expanding India’s MRI Footprint

With only around 5,000 MRI machines for a population of 1.4 billion, India has a vast gap in diagnostic infrastructure. Voxel Grids currently has the capacity to manufacture 20 to 25 MRI scanners annually at its Bengaluru facility. While this scale is modest compared to global manufacturers, it is a crucial first step.

The company is also working on developing a containerised mobile MRI unit. This could revolutionise healthcare delivery in remote and underserved areas, bringing advanced diagnostics directly to communities that have never had access to such services.

Creating a Domestic Med Tech Ecosystem

Beyond the immediate impact on patients, Voxel Grids’ success is helping to build a domestic med tech ecosystem. The company says nearly every component of its MRI scanner, from hardware and electronics to imaging software, has been designed in-house. Prime Minister Narendra Modi has highlighted India’s growing self-reliant medical technology ecosystem, noting that more than 50 high-end medical devices have started being manufactured in India under the PLI scheme.

The Indian medical device sector is currently valued at around $14 billion and is projected to reach $30-50 billion. The MAHA Med Tech Mission aims to make India a global hub for AI-driven medical tech exports. Industry experts say that geopolitical shifts and ongoing supply chain disruptions have made Med Tech manufacturing a strategic priority for India.

A Model for Other Developing Nations

The Voxel Grids story offers lessons for other developing nations that are seeking to build their own med tech capabilities. It shows that with determination, innovation, and the right support, it is possible to break into industries that have long been dominated by a handful of global giants.

It also shows the importance of keeping critical technology in domestic hands. Had Voxel Grids sold to a multinational corporation, India would have lost not just a company but the opportunity to build a domestic med tech ecosystem. The Tata-Zoho intervention ensured that this did not happen.
